Nurse Practitioner

We are looking for superstar Nurse Practitioners or Physicians Assistants to be part of a facilities-based team of medical providers who provide medical services to patients living in long-term care facilities.While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and well-being of the elderly population.If you're dedicated to advancing your knowledge and clinical expertise through research and professional discourse, we'd like to meet you**Job Qualifications**:Current, unrestricted NP or PA license.DEA licensure.Ability to manage patients with advanced illness and multiple chronic conditions.**Responsibilities include but are not limited to**:Prescribe appropriate medications.Order, review, and/or follow up with treatments and diagnostic testing.Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and/or the care plan.Collaborate with primary care physician.Provider will assess and monitor residents on a regular basis.
