**¿Qué harás?**
Ayudarás a pacientes con demencia y sus cuidadores mediante una plataforma innovadora con inteligencia artificial. Como Care Coordinator, serás el enlace principal para brindar apoyo, coordinar soluciones y conectar recursos, reduciendo la carga del cuidador.

**Responsabilidades**

Registro de pacientes y cuidadores en la plataforma.
Comunicación constante (chat, llamadas grabadas).
Coordinar recursos clínicos y comunitarios.
Uso de herramientas digitales y AI para apoyo y seguimiento.
Documentación segura (cumplimiento normativo).

**Requisitos**
Formación en Enfermería o Trabajo Social.
Experiência mínima de 3 años en atención a pacientes o servicio al cliente en salud.
Conocimiento del sistema de salud en EE. UU.
Inglés avanzado
Manejo de tecnología y plataformas digitales ia.

Experiência con población geriátrica/demencia deseable.
