• Conduct comprehensive medication reviews to identify, prevent, and resolve drug-related problems.
• Collaborate with healthcare professionals, patients, and caregivers to develop and implement individualized pharmacotherapeutic plans.
• Monitor patient response to therapy, adjust medication regimens as needed, and document all interventions and recommendations.
• Provide patient education on medication use, potential side effects, and disease management strategies.
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
• Maintain accurate and up-to-date patient records in accordance with professional standards and regulatory requirements.
• Stay abreast of the latest advancements in pharmaceutical care and evidence-based medicine.
• Utilize telehealth platforms and electronic health records effectively to deliver remote patient care.

• Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
• Active pharmacist license in Mexico.
• Board Certification in Pharmacotherapy (BCPS) or equivalent experience is highly preferred.
• Minimum of 3-5 years of clinical pharmacy experience, with a strong emphasis on pharmacotherapy management.
• Proven experience in medication therapy management (MTM) and chronic disease state management.
• Excellent communication, interpersonal, and critical thinking skills.
• Proficiency with electronic health record (EHR) systems and other relevant healthcare technologies.
• Ability to work independently, manage time effectively, and meet deadlines in a remote work environment.
• Demonstrated commitment to patient advocacy and a patient-centered approach to care.
