We are currently in search of an **Onboarding Specialist** to push the boundaries and join our team of hard-working, driven individuals. The omission of specific statement of duties does not exclude them form the position if the work is similar, related or is a logical assignment to the positon.- Work details are provided in the MED B tracker- MED B tracker is the dashboard for this role- Based on information in tracker (and training provided) enter data provided in tracker into Infinite Database and RainTree accurately and within same day- Update tracker immediately as change happens or new information is received- Follow up with patients MD to receive MD order/prescription so patient can receive services asap. Follow up timely as per timelines given- Responsible to get Discharge Confirmations from the referring agency according to set timelines given- Excellent communication with team members including the Business Development Liaison and/or referral source to update on patient status etc.- Communicate timely and appropriately with team members and partners to ensure smooth delivery of service- Monitor referrals received in RainTree Fax throughout the day and forward to appropriate Business Development Liaison/Director- Run Weekly reports on overdue Plan of care on RainTree and document progress in spreadsheet.- Responsible for Plan of Care are received follow up on per timeline given by Manger of Operations- Participate in team meetings**Work Environment**:As the COVID-19 pandemic and the different levels of quarantine continue to threaten our future, what remains isour commitment to our people.
