Job Summary

We are seeking a skilled and compassionate Pediatric Home Health Nurse to join our team at Connect Pediatrics. As a Pediatric Home Health Nurse, you will provide high-quality, patient-centered care to children in the comfort of their own homes.

Responsibilities
  • Provide skilled nursing care to pediatric patients in their homes, following physician-prescribed plans of care.
  • Administer medications, feedings, and treatments as ordered by the physician.
  • Maintain accurate and detailed records of patient care, including real-time charting.
  • Collaborate with clinical leadership to coordinate patient care and ensure seamless transitions.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
Requirements
  • Current nursing license (LVN or RN) to practice in the state of Texas.
  • Current CPR Certification.
  • Pediatric experience preferred, but not required. We provide comprehensive training and support to ensure our nurses are equipped to provide exceptional care.
