**Position Overview**:
We are guided at all times by the values of inclusion, cultural sensitivity and genuine caring. A Document Coordinator is responsible for receiving, processing and submitting home care clinical therapy documents in a timely manner, as well as reviewing all documents for accuracy and completeness.

**Primary Responsibilities**:

- Receives, processes and submits home care clinical therapy documents in a timely manner.
- Reviews all documents for accuracy and completeness.
- Additional Responsibilities may include:

- Provide education to providers as needed regarding documentation.
- Submits needed documents to Home Care agencies when requested.
- Processes invoices to Home Care agencies.
- Provides coverage for other members of the home care team as needed.
- Provides other related duties as assigned and requested.
