**About the Role**

We are currently seeking a dedicated and compassionate Patient Coordinator to join our growing team. As a Patient Coordinator at Assistantly, you will play a pivotal role in ensuring the seamless coordination of patient care within the homecare setting. This position involves a range of responsibilities including case management, timesheet processing, attendance management, caregiver evaluation, and facilitating communication between caregivers, patients, their families, and insurance providers. The Patient Coordinator serves as a key liaison between caregivers and the nursing department, ensuring effective collaboration and adherence to care plans.

**Responsibilities**:

- Oversee the coordination of patient cases, ensuring that all aspects of care are managed efficiently and effectively.
- Ensuring caregivers are clocking in and out from patients' phones utilizing the approved methods.
- Ensuring caregivers are completing all tasks listed on the patients POC, if a task is not completed, a refusal code is entered.
- Manage the accurate and timely processing of caregiver timesheets to ensure proper compensation and billing.
- Maintain records of caregiver attendance and address any attendance-related issues promptly and professionally.
- Evaluate caregiver performance and provide constructive feedback to ensure the delivery of high-quality care.
- Review and discuss the patient's plan of care with caregivers, ensuring alignment with the patient's needs and preferences.
- Foster open communication channels between caregivers, patients, their families, and other stakeholders to facilitate the exchange of vital information and address any concerns or questions.
- Liaise with patients' insurance providers to provide updates on patient conditions and ensure that necessary approvals and authorizations are obtained.
- Maintain open communication with the nursing department to coordinate patient care activities, share relevant information, and address any care-related issues or concerns.

**Requirements**:

- Prior experience as a Patient Coordinator or in a similar role within a healthcare setting.
- Exceptional customer service skills with a compassionate and caring demeanor.
- Strong organizational and multitasking abilities, with the ability to prioritize tasks effectively.
- Excellent communication skills, both verbal and written, with attention to detail.
- Ability to work well in a fast-paced environment and remain calm under pressure.
- High level of professionalism and adherence to patient confidentiality regulations.
- Strong problem-solving skills and ability to find solutions to patient inquiries and concerns.
