We are seeking a skilled Medical Provider to join our team of healthcare professionals at Longterm Health Management Services. This is an excellent opportunity to make a positive impact on the lives of elderly patients in long-term care facilities.

**Job Responsibilities:**

The ideal candidate will have strong clinical expertise and be able to manage patients with advanced illness and multiple chronic conditions. Key responsibilities include:

  • Prescribing appropriate medications and ordering diagnostic testing as needed
  • Collaborating with primary care physicians and nursing home staff to discuss changes in condition and/or the care plan
  • Assessing and monitoring residents on a regular basis, ensuring their needs are met and they receive high-quality care

**Requirements:**

  • Current, unrestricted NP or PA license
  • DEA licensure
  • Strong communication and interpersonal skills

**What We Offer:**
