Patient Experience Navigator

The Patient Engagement Representative plays a vital role in creating a positive patient experience by effectively handling outreach to patients. This position focuses on elevating the patient experience, stimulating engagement, and streamlining communication. Key responsibilities include providing comprehensive assistance, guidance, and unwavering support to patients, ensuring a positive and fulfilling healthcare journey.
- Conducting Outreach - Gather feedback, assess needs, and identify areas for improvement through patient outreach.
- Guidance and Support - Assist patients navigating the healthcare system, including appointment scheduling, insurance inquiries, and access to resources.
- Member Support - Respond and assist with Primary Care Provider (PCP) allocation.
- Tech-Enhanced Engagement - Utilize technology and digital platforms to enhance patient engagement initiatives.
- Queue Monitoring - Ensure patient inquiries are handled professionally and in a courteous, responsive, and timely manner.
- Knowledge Updates - Stay informed about best practices and trends in patient engagement, healthcare technology, and patient-centered care.
- Interpretation Services - Arrange interpretation services for members as needed.
- Education - High School Diploma or equivalent required.
- Experience - Previous experience in a healthcare setting, preferably in patient engagement, patient advocacy, or healthcare coordination, with 2-3 years previous healthcare experience preferred.
- Skills and Abilities - Excellent communication and interpersonal skills, with the ability to interact effectively with patients, staff, and healthcare providers.
- Knowledge and Abilities - Strong interpersonal skills, empathy, and compassion for patients and their families, with a commitment to providing patient-centered care.
- Technical Skills - Proficiency in utilizing technology and digital platforms for patient engagement initiatives, flexibility, and adaptability to meet changing needs and priorities.
This role requires a professional office environment, with a focus on non-patient-facing duties and full-time rotation between office and remote work. The ideal candidate will possess excellent communication and interpersonal skills, with a commitment to providing patient-centered care.
