**Care Coordinator - Job Summary**:
The Care Coordinator’s primary objective is to assist PROVE’s medical provider network and Case Managers by delivering exceptional customer service. The Care Coordinator will be responsible for ensuring the efficient and timely completion of provider-related tasks related to a patient’s treatment. The Care Coordinator is responsible for confirming patients have been scheduled, collecting referrals, records, and bills post-appointment, and confirming that all pertinent information has been gathered at the completion of treatment.
Care Coordinators are personable, as this is a customer service role. They should be able to manage tasks in a timely and effective manner while multitasking and ensuring quality completion. Care Coordinators work closely with PROVE’s case management, processing, and contracting teams to ensure medical treatment is scheduled, completed, and funded promptly. It is not uncommon for Care Coordinators to manage 80+ active tasks a day, so they must possess exceptional organizational, time management, and multitasking skills. The Care Coordinator is expected to be well-versed in and comfortable explaining PROVE’s processes. They also build and maintain relationships with our network of medical providers. Although this role is primarily remote, occasional travel to events may be required.

- Proven track record of exceeding objectives in previous roles
- Ability to manage and complete tasks efficiently to support Case Managers
- Ability to navigate obstacles and keep the process moving forward
- Expertise in PROVE’s provider processes
- Innovate Care Coordinator processes to align with current business needs and PROVE's vision
- Strong phone etiquette and ability to convert initial conversations into lasting relationships
- Ability to guide medical providers through PROVE’s processes
- Excellent time management skills, with the ability to work independently and maintain a high level of activity and effectiveness
- Experience using various software platforms
- Strong attention to detail
- Strong organizational and multitasking skills
**Education Qualifications**:
- Associate’s degree or equivalent preferred
**Experience Qualifications**:
- Fluent in English and Spanish
- 2+ years’ experience in an administrative or operations role
- 2+ years’ experience in Customer Service
- Fluency in medical terminology and knowledge of various medical disciplines is preferred
- Patient scheduling experience is preferred
- Knowledge of healthcare or the personal injury space is preferred
Salary Range: $1,000 - $1,500 USD per month, depending on experience.
Paid Time Off (PTO) available.
