We are seeking a dedicated Service Coordinator to join our team at Volunteers of America Colorado. This is an exciting opportunity to make a meaningful difference in the lives of seniors and individuals with disabilities living in our affordable housing portfolio.
About the RoleThe successful candidate will be responsible for providing comprehensive support services, including needs-based assessments, case management, and connection to income resources and public benefits. You will work closely with our Community Administrator to ensure residents receive the necessary assistance to live independently and age in place.
Key Responsibilities- Conduct thorough needs-based assessments to identify resident requirements and develop personalized plans for service delivery.
- Provide ongoing case management support to residents, facilitating access to essential resources and services.
- Collaborate with local service providers and multi-disciplinary teams to ensure seamless service delivery and coordination.
- Develop and facilitate health, wellness, and educational programs tailored to resident needs, promoting improved wellbeing and community engagement.
- Maintain accurate records and documentation, adhering to HUD and VOA guidelines.
- Bachelor's degree in human services, social work, or a related field, or equivalent experience.
- Minimum one year of experience working with seniors and/or individuals with disabilities.
- Ability to obtain first aid, automated external defibrillator, and cardiopulmonary resuscitation certifications.
- Excellent interpersonal and organizational skills, with proficiency in using Windows environment and databases.
- Mental health experience preferred.
- Bilingual skills in a language represented in the resident community (Mandarin Chinese, Vietnamese, or Spanish) are an asset.
Volunteers of America Colorado offers a competitive salary range of $42,000 - $45,000 per year, commensurate with experience, as well as a comprehensive benefits package, including:
